How do I obtain a copy of my incident report or accident report?
You can do a walk-in request at the Police Dept or at City Hall.
You can do a telephone request at 706-348-7078
You can email our request form to cpdinfo@cityofclevelandga.org
You can fax our request form to 706-865-9124
Please allow up to three (3) business days after day of request to get your report.
There is a $5 fee for all reports.
Reports will need to be picked up at City Hall.
If you need the report mailed, emailed, or faxed, the $5 fee has to be paid before it wil be sent out.
How do I obtain a Criminal History?
Visit the White County Sheriff's Office:
Monday - Friday from 8 A.M. to 5 P.M.
Consent form signed in person
Need a copy of driver's license
Fee: $10.00 Cash Only
